PAPERWORK

It is recommended that you complete the patient intake forms before your scheduled appointment. These forms contain important information about you and your injury that will help the healthcare provider better understand your medical history and current condition. By filling out the forms in advance, you enable the healthcare provider to dedicate more time to providing you with high-quality care during your appointment, rather than spending time collecting basic information.

If you prefer not to complete the forms beforehand, you can also arrive 15 minutes before your appointment to fill them out in person. However, completing the paperwork ahead of time is a convenient way to ensure a smoother and more efficient process during your visit. Your cooperation in this matter will contribute to a more effective and personalized healthcare experience.

Consultation

In order to have a better understanding of your injury the healthcare provider will go through a detailed history, and physical exam. The provider will review the findings, give a diagnosis and discuss potential treatment options.

X-Ray Or Imaging Studies

If x-rays or other studies are required for further evaluation of your injury, the healthcare provider will inform you and provide you with a prescription to have those completed at a facility close to you.

Care Instructions

Along the course of treatment, patients will be given instructions on home exercises and/or stretches. This may include certain therapy applications (i.e. ice, heat, contrast, baths, essential oils, etc.), certain activities or positions to avoid, as well as the frequency of care.
